Henry V memorial coin?

Many years ago I bought a coin and have never been able to figure out what it really is. Quite heavy and appears to be three pieces, a band around the edge

Front has a picture of Henry V

Words are : Henricus . V . D. G. ANG. FR. ET. HIB.REX

shown with a crown, suit of steel

Back has angel/reclining figure and death? mask

Natus 1320

Coronat 1320

Mort 1422

HENRY V, MEMORIAL 1422

DASSIER, Jean: England, 1731, Silver, 41 mm

Obv: Bust of Henry V HENRICUS. V. D.G. ANG. FR. ET. HIB. REX.

Rev: Henry, amid captured arms, is reclining on monument in form of a sarcophagus. Fame is crowning him and sounding his praises. NATUS 1388 CORONAT. 1413 MORT. 1422.

Rare in silver

Ref: Med Ill i, 12/6; Eimer 28/16; Eisler 257/16a; Thompson 26/14
