The Victorians were great ones for decorating coins - there are any number of enamelled coins out there. This looks more unusual - like someone has stuck a thin (probably metal) disc over the obverse, within the rim. The central hole was probably to suspend the coin using a string or wire or something similar.

Im guessing its only worth a couple of quid but ive never seen one for sale

Unlikely - though you never know, if you were to put it on eBay. The hole in the middle lets it down a lot though.
